Same-day ART study presented as an Oral Abstract at AIDS 2018 in Amsterdam

High attrition during both pre- and post-ART initiation phases impedes the progress of Thailand in ending AIDS by 2030, and multifactorial causes have contributed to this phenomenon. Based on a survey conducted by Bangkok Metropolitan Administration in 2017, almost half of people diagnosed with HIV infection in Bangkok did not seek treatment due to convoluted processes in transferring them to their eligible hospitals. Additionally, perceived patient readiness has been a critical element for low ART uptake. To address these concerns, the Thai Red Cross Anonymous Clinic began offering Same-Day ART service since July 2017, hoping to increase ART uptake after HIV diagnosis and linkage to care.

The results from July, 2017 to April, 2018, were presented by Pich Seekaew at the International AIDS Conference 2018 (AIDS 2018) in Amsterdam, the Netherlands, in an Oral Abstract session Pedal to the Metal: Accelerating the cascade session. The study found that 90.4% of the 1,589 diagnosed HIV-positive persons accepted the service, and among these, 79.4% received ART on the same day of HIV diagnosis and 89.6% were successfully referred to long-term ART care. We also found that, when compared to standard of care at the Thai Red Cross Anonymous Clinic, Same-Day ART clients were 3.9 times more likely to start ART after HIV diagnosis and 2.2 times more¬ likely to be virally suppressed. This shows that Same-Day ART is safe and feasible to implement in HIV testing center. However, more data are still needed to investigate its applicability and generalizability in diverse settings. We have discussed the possibility with public health and community partners in Chiang Rai, Chiang Mai, Ubonratchathani and Songkhla who are ready to launch the Same-Day ART pilot in their provinces later this year.
